Borrowing the mythology of Game of Thrones, the chapter confronts one of the most serious risks of AI in healthcare: algorithmic bias born from training data that does not represent the whole population, reproducing and sometimes amplifying existing inequalities. The authors call for governance structures as vigilant as a "Night's Watch", and for explainability (XAI) as an indispensable tool so that physicians and patients can understand, and question, an algorithm's recommendations before trusting them blindly.
